Rachel Forman, MD

Dr. Rachel Forman is a stroke neurologist and Assistant Professor of Neurology at Yale New Haven Hospital. Her areas of interest include community and childhood stroke education, community and patient partnership, stroke prevention, and post-stroke blood pressure. She serves as the faculty advisor for the Yale Stamp Out Stroke program whose mission is to educate the local community about stroke. She also serves as the Yale site PI for the REDUCE trial, is on the CT Board of Directors for the AHA, and runs the Yale Patient and Family Advisory Board.
